The following services are not covered by Ontario Health Insurance Plan(OHIP) and patient must pay for these services before the appointment:
- TB Skin Test:
- General:
- A blank form should be provided by patients and staff should verify the purpose.
- Work/Volunteer – not coverd
- School/Placement – covered (patient only pays for documentation)
- Canadian Immigration (TB medical surveilance form) – covered (patient only pays for documentation)
- Prices:
- 1- Step TB skin test: TB testing – $30, TB reading – $20 (done after 48-72 hours)
- 2-Step TB skin test:
- First step: TB testing – $30, TB reading – $20 (done after 48-72 hours)
- Second step: Same process and fee as the first step – should be done within 7-21 days after the first step.
- Documentation:
A blank form should be provided by patients and staff should verify the purpose- Immunization form – $60 (ONLY documentation of immune status for MMRV, HEP B, Vaccination and TB)
- TB form (exclusive TB form)
- Covered only for Health-related school programs, continuation of school, daycare forms.
- $20 (TB testing alone, without documentation of immune status for MMRV, HEP B and vaccinations for employment and under than above)
- For patients without health insurance/paying out of pocket:
- TB testing – $40
- TB reading – $40
- Documentation fees as above
- Other cases:
- TB test requested by ministry of health program (e.g., public health) – OHIP covered
- Tb test requested for admission or continuation in a day care, pre-school, school, college, university or other educational institution – Test covered by OHIP; documentation/form/report completion is not covered. Charge: $20
- Tb test requested for employment/volunteer purpose (test + reading) – not covered. Charge: $50 per step
- General:
- Immunization form (including TB and documentation of immune status for MMRV, HEP Band vaccinations): $60
- Vaccine* administration fee (uninsured/out-of-country resident patients): 30$ per injection
- Completion of form physicals pre-employment certification of fitness/fitness clubs or nursing/hospital/home employee: $35 & Up
- General back to work note/sick note/day care note:
- for OHIP, other provinces (except QC) and IFH: 30$
- for UHIP and uninsured patients: 50$

Our walk-in clinic team consists of Family physicians and Chiropractors. In addition, we provide travel consultation including vaccinations and tests required before traveling, TB(tuberculosis) test, drivers full physical test, immunization record, and full immunization, skin tag/wart removal.
